Iran Releases Rare Video of Supreme Leader Mojtaba Khamenei Amid Claims He Was Badly Wounded and His Appearance Altered by US-Israeli Strikes

Iran's state media released a rare video on Sunday showing Supreme Leader Mojtaba Khamenei alive, following months of reports claiming he was gravely wounded and his appearance altered by strikes carried out by the United States and Israel. The clip, distributed by the semi-official Mehr News Agency, ran under fifteen seconds and showed Khamenei speaking with a group of men, though it carried no date or location and kept parts of his face obscured.

Iran also confirmed on Sunday that Khamenei had met with President Masoud Pezeshkian, who is beginning his third year in office. It is not the first such release: Iran's state broadcaster IRIB had earlier published undated footage showing him teaching a religious class, similarly without disclosing when or where it was filmed.

A Leader Not Seen Since His Elevation
Mojtaba Khamenei was selected as Iran's Supreme Leader in early March 2026, after his father, Ali Khamenei, was killed in a US-Israeli airstrike on February 28 during the 2026 Iran war. Since his selection by the Assembly of Experts, he has not been seen in public, a prolonged absence that has fuelled sustained speculation about his health, whereabouts, and even whether he remains capable of governing.

Reports have claimed Khamenei's appearance "changed significantly" as a result of the strike on the Supreme Leader's compound in central Tehran, and that he sustained a serious injury to one or both legs, according to Reuters reporting from April. CNN has separately reported, citing sources familiar with his selection process, that Khamenei suffered a fractured foot and minor facial injuries during the opening day of the US-Israeli bombing campaign. Opposition-linked Iranian outlets and some Israeli media have advanced further claims about his health and alleged hospitalization, though none of these accounts have been independently verified.

In July, U.S. President Donald Trump said in a Fox News interview that Mojtaba Khamenei was "close to death" as a result of his injuries.

A President Who Struggles to Reach Him
President Pezeshkian's own account of his access to the Supreme Leader has shifted in recent weeks, deepening rather than resolving the uncertainty. On July 21, he said interactions with Khamenei were "increasing day by day," but by early August he described communicating with him as "very difficult". State media reported that the late-July meeting between the two men covered military affairs and the economy, including resource security, foreign exchange management, and energy use, as well as the country's cost-of-living pressures and the broader outlook for the conflict.

The ambiguity has not gone unnoticed. Commentary circulating around the video's release has pointed out that Pezeshkian previously said he met the Supreme Leader "in the dark" and was not even certain it was him feeding open doubt in some quarters about the extent of Khamenei's condition and role.

A Pattern of Undated, Partial Footage
Iranian state media's approach to proving Khamenei's survival has followed a consistent pattern: short clips, no verifiable date or location, and, in the latest release, a face only partially shown. Officials have signaled more footage may follow a paramilitary leader has promised additional images of Khamenei in the future but each release so far has done more to sustain questions than settle them.

For a state built around the authority and visibility of its Supreme Leader, the extended absence of Mojtaba Khamenei from public life replaced instead by carefully controlled, undated glimpses marks an unusual chapter in the Islamic Republic's history, one unfolding against the backdrop of a leadership succession forced by war rather than the ordinary passage of time.
